    PURPOSE:

    To study the relationship between retinal microcirculation and serum adiponectin, an important adipocytokine secreted by adipocytes, concentrations in patients with type 2 diabetes mellitus.
    METHODS:

    Using a laser Doppler velocimetry system, we simultaneously measured the retinal blood flow (RBF) values and retinal vessel diameter and blood velocity in 64 consecutive Japanese patients (mean age ± SD, 59.8 ± 10.4 years) with type 2 diabetes with no or mild nonproliferative diabetic retinopathy. We compared the values with the RBF and serum adiponectin concentrations in these patients. The patients were divided into two groups based on sex (33 males, 31 females).
    RESULTS:

    The plasma adiponectin concentrations were correlated positively with the retinal vessel diameter (r = 0.480; P = 0.005), retinal blood velocity (r = 0.399; P = 0.02), and RBF (r = 0.518; P = 0.002) and correlated negatively with the retinal arterial vascular resistance (r = -0.598; P = 0.0002) in males, but not females, with type 2 diabetes with early-stage diabetic retinopathy. Multiple regression analysis showed that the plasma adiponectin level was independently and positively correlated with RBF and negatively correlated with retinal arterial vascular resistance.
    CONCLUSIONS:

    Our results indicated that a high concentration of serum adiponectin may be associated with increased RBF, probably via the increased blood velocity and dilated vessel diameter in males with type 2 diabetes with early-phase diabetic retinopathy.

    The aim of the current pilot study is to investigate the efficacy of a novel enhanced vitreous imaging (EVI) in primary macular holes (MHs) using a spectral-domain optical coherence tomography (SD-OCT). Thirty-four eyes of 32 consecutive patients with a MH were examined in one time cross-sectional study. The vitreomacular interface was assessed using SD-OCT with conventional and EVI technique. Twenty-three of the 34 eyes did not show a Weiss ring, and in 22 of those, we observed a MH with an open roof or operculum and a detached posterior vitreous cortex with conventional vitreous imaging. Using EVI-OCT, we visualized the reflection of the posterior vitreous with a vitreopapillary attachment. One of the 23 eyes without a Weiss ring had a central round retinal defect without an operculum, and the conventional SD-OCT showed an empty vitreous, suggesting a complete posterior vitreous detachment. However, the EVI-OCT revealed the reflection of the posterior vitreous, and the cortex appeared to still be completely attached. In all the 23 eyes without a Weiss ring, EVI-OCT detected the reflection of the posterior vitreous and vitreopapillary attachment. In all 11 eyes with a Weiss ring (stage 4 hole), EVI-OCT showed an optically empty space in the posterior vitreous cavity without a vitreopapillary attachment. EVI-OCT may be a new reliable method for preoperative evaluations to determine the presence or absence of a complete posterior vitreous detachment in macular diseases with an indistinct Weiss ring.

    PURPOSE:

    To investigate the effect of intravitreal bevacizumab (IVB) injections for macular edema secondary to acute branch retinal vein occlusion on the retinal microcirculation.
    METHODS:

    The study was a prospective, interventional case series. Central macular thickness using spectral-domain optical coherence tomography and retinal blood flow (RBF) in untreated eyes with macular edema secondary to acute branch retinal vein occlusion in occluded (V1) and opposite venules in affected eyes (V2) and the equivalent venules in contralateral eyes (V3), using laser Doppler velocimetry during follow-up and after IVB injection, were measured.
    RESULTS:

    In 33 eyes with acute branch retinal vein occlusion of <2 months of duration at the first visit, changes in the retinal microcirculation for 1 month was observed; the macular edema improved spontaneously, and the RBF was unchanged in 15 of 33 eyes, and the RBF increased by 23.3% in 18 eyes with persistent macular edema. Twenty-four eyes received an IVB injection (1.25 mg per 0.05 mL). The RBF did not change significantly during follow-up. In 8 of 24 eyes (33%) with improved macular edema 3 months after the treatment, the average RBF values before injection were significantly higher compared with that of eyes with recurrent edema.
    CONCLUSION:

    One IVB injection might have little effect on the retinal microcirculation in patients with macular edema secondary to acute branch retinal vein occlusion at least 3 months after the injection. However, the increased RBF in the occluded venules before injection might be associated with improved macular edema after the IVB injection.

    Although sphingosine 1-phosphate (S1P), a bioactive lipid derived from activated platelets, has a variety of physiologic effects on vessels, no reports have described the effect of S1P on the retinal circulation. We examined the effect and underlying mechanism of the vasomotor action of S1P on porcine retinal arterioles. The porcine retinal arterioles were isolated, cannulated, and pressurized without flow for in vitro study. S1P-induced diameter changes were recorded using videomicroscopic techniques. S1P elicited concentration-dependent (1 nM-10 μM) vasoconstriction of the retinal arterioles that was abolished by the S1P receptor 2 (S1PR2) antagonist JTE-013. S1P-induced vasoconstriction was abolished by the Rho kinase (ROCK) inhibitor H-1152 and was inhibited partly by the protein kinase C (PKC) inhibitor Gö-6983. The inhibition of phospholipase C by U73122 and L-type voltage-operated calcium channels (L-VOCCs) by nifedipine inhibited S1P-induced vasoconstriction; a combination of both inhibitors abolished S1P-induced vasoconstriction. Furthermore, inhibition of myosin light chain kinase (MLCK) by ML-9 significantly blocked S1P-induced vasoconstriction; further coadministration of ML-9 with H-1152 or Gö-6983 abolished S1P-induced vasoconstriction. The current data suggest that S1P elicits vasoconstriction of the retinal arterioles via S1PR2 in vascular smooth muscle cells and this vasoconstriction may be mediated by the Ca2+ -sensitive pathway via activation of PKC leading to activation of ROCK and the Ca2+ -dependent pathway via activation of L-VOCCs resulting in activation of MLCK.

    PURPOSE:

    To investigate the regulatory mechanisms responsible for autoregulation of retinal blood flow (RBF) during periods of decreased ocular perfusion pressure (OPP).
    METHODS:

    The effects of acute reductions in OPP on RBF were assessed using laser Doppler velocimetry in cats. The OPP decreased from 90 to 40 mm Hg by increasing the IOP (elevated IOP) or by decreasing the systemic blood pressure via exsanguination (systemic hypotension). The contributions of nitric oxide (NO), adenosine, and/or N-methyl-D-aspartic acid (NMDA) in regulation of the retinal arteriolar hemodynamics during decreased OPP was determined at 120 minutes after intravitreal injection of various inhibitors or PBS.
    RESULTS:

    Following PBS injection, the flow velocity decreased in proportion to the decrease in OPP; however, the retinal arteriolar diameter gradually increased. Consequently, the RBF was maintained near baseline levels when the OPP exceeded 70 mm Hg but decreased significantly (P < 0.01) when the OPP fell to less than or equal to 60 mm Hg due to elevated IOP or systemic hypotension. Adenosine receptor blocker 8-(p-sulfophenyl)theophylline, significantly (P < 0.01) enhanced decreases in RBF induced by elevated IOP and systemic hypotension at OPP from 80 to 40 mm Hg, whereas NO synthase inhibitor N(G)-nitro-L-arginine-methyl ester and NMDA receptor antagonist DL-2-amino-5-phosphonopentanoic acid only significantly (P < 0.01) enhanced reductions in RBF induced by elevated IOP.
    CONCLUSIONS:

    These results indicate that adenosine contributes to autoregulation of RBF during systemic hypotension, whereas adenosine, NO, and NMDA receptors autoregulate the RBF after elevated IOP. Different vasoregulatory factors might contribute to autoregulation of RBF after decreases in OPP induced by elevated IOP and systemic hypotension.

    PURPOSE:
    To investigate the relationship between retinal fractal dimensions (Dfs) and retinal circulation, which is impaired in early-stage diabetic retinopathy (DR) in patients with type 2 diabetes mellitus (DM).
    METHODS:
    Using a laser Doppler velocimetry system, we measured the retinal vessel diameter (D) and blood velocity (V) and calculated the retinal blood flow (RBF) in the retinal arterioles and venules in 106 eyes (106 patients, mean age ± standard deviation, 58.7 ± 9.8 years). Patients with type 2 DM had no (n = 86) or mild nonproliferative DR (n = 20). The Dfs were measured on the retinal photographs using a semiautomatic computer-based program.
    RESULTS:
    The average D, V, and RBF in the retinal arterioles and venules were, respectively, 107.9 ± 13.3 and 139.4 ± 20.1 µm, 33.2 ± 7.0 and 22.6 ± 6.1 mm/s and 9.3 ± 2.9 and 10.4 ± 3.8 µl/min. The average Df was 1.4276 ± 0.0193. There was no association between the Df and any retinal circulatory parameters of the retinal arterioles. In contrast, we found significant correlations between the Df and the vessel D (r = 0.37, p = 0.0002) and RBF (r = 0.22, p = 0.026) in the retinal venules.
    CONCLUSION:
    The Df might be associated with changes in the retinal circulation in patients with type 2 DM.

    PURPOSE:

    Adiponectin, an important adipocytokine secreted by adipocytes, has anti-inflammatory and atheroprotective effects on vascular tissue via the adiponectin receptor (adipoR). However, the action of adiponectin in the retinal microcirculation is unknown. We examined the direct effect and underlying mechanism of the vasomotor action of adiponectin in porcine retinal arterioles.
    METHODS:

    Porcine retinal arterioles (internal diameter, 60-90 μm) were isolated, cannulated, and pressurized (55 cmH2O) without flow in this in vitro study. Videomicroscopic techniques were used to record changes in diameter in response to adiponectin.
    RESULTS:

    The retinal arterioles dilated in a dose-dependent (0.125-7.5 μg/mL) manner in response to adiponectin. The vasodilation decreased significantly after removal of the endothelium. N(G)-nitro-L-arginine methyl ester (a nitric oxide [NO] synthase inhibitor), 1H-1,2,4-oxadia-zolo[4,3-a]quinoxalin-1-one (a soluble guanylyl cyclase inhibitor), but not wortmannin (a phosphatidylinositol 3-kinase inhibitor) inhibited the effect of adiponectin-induced vasodilation comparable with that of denudation. Pretreatment with compound C, an activated protein kinase (AMPK) inhibitor, partially but significantly reduced vasodilation. Incubation with GW6471, a peroxisome proliferator-activated receptor blocker, did not significantly inhibit vasodilation by adiponectin. AdipoR1 and adipoR2 immunoreactions were observed in the endothelium of retinal arterioles.
    CONCLUSIONS:

    Adiponectin elicits mainly endothelium-dependent dilation of the retinal arterioles. Endothelium-dependent vasodilation likely induced by adiponectin results from NO via activation of guanylyl cyclase that is partially dependent on AMPK activity. Understanding the effect of adiponectin on the retinal vasculature may help improve potential therapies for retinal vascular disorders, especially diabetic retinopathy in patients with type 2 diabetes mellitus.

    我々の臨床研究の結果から、2型糖尿病網膜症では発症前あるいは発症早期では網膜血流が低下するという結果が得られており、網膜微小循環障害が糖尿病網膜症の病態に関与していると考えられる。網膜微小循環調節には血管拡張因子と収縮因子がバランス良く関与することが重要であるが、糖尿病ではこのバランスが崩れることが報告されている。具体的には、重要な血管拡張物質である一酸化窒素(NO)の血管内皮からの産生が早期から低下することが示されており、さらに酸化ストレスが増加するとNOはフリーラジカルとして血管障害性に働き、網膜血管内皮機能の低下は、白血球接着増加を介して糖尿病血管障害を悪化させると考えられる。一方、強力な血管収縮因子であるエンドセリン(ET)の産生が糖尿病で増加すると報告されており、これが血流障害に関与すると考えられる。また、血管内皮増殖因子VEGFやレニンーアンジオテンシン系も糖尿病網膜症の病態として重要な因子であり、糖尿病網膜症患者での硝子体中濃度の増加が報告されているため、網膜循環への影響についても考慮されなければならない。
     微小循環障害の側面から糖尿病網膜症の病態を捉えることにより、新しい治療法開発につながることが期待される。本講演ではその可能性について考えてみたい。

    網膜光凝固術や硝子体手術、眼局所薬物治療など糖尿病網膜症に対する治療法の進歩にもかかわらず、未だ年間3000人もの糖尿病網膜症患者が失明に至るという事実は、眼局所での治療法のみでは限界があることを示しており、糖尿病網膜症の治療には、眼局所因子のみならず全身因子の影響も十分考慮する必要がある。最近の大規模臨床研究の成果から、高血圧治療薬であるアンジオテンシン変換酵素(ACE)阻害剤 (UKPDS study)に加えてアンジオテンシンII受容体拮抗薬(ARB)カンデサルタン(DIRECT study)や脂質異常症治療薬フィブラート(FIELD study)も糖尿病網膜症の抑制効果を有することが示され、眼科的治療のみならず薬物内服療法を加え、全身因子の影響を考慮した集学的治療を行う必要がある。
     これまで我々は、我が国の糖尿病患者の大部分を占める2型糖尿病患者を対象として、レーザードップラ眼底血流計を用いて非侵襲的に網膜循環を評価し、網膜動脈血流量が糖尿病網膜症発症前からすでに低値であること、血中LDLやクレアチニンが網膜血流低下に関与することを報告した(IOVS, 2010)。さらに腎機能と網膜循環との関連に着目して解析を進め、早期の網膜症および腎症を有する2型糖尿病では、腎機能障害が進行すると網膜血流も低下していることを見いだした(Diabetes Care, 2013)。そこで我々は、眼循環障害を改善させる薬剤を探索し、新しい網膜硝子体疾患治療法の開発につながることが期待され、実際我々は摘出血管を用いたex vivo実験系で、脂質異常症改善薬スタチン(IOVS, 2007)およびフェノフィブラート(IOVS, 2012)、インスリン抵抗性改善薬ピオグリタゾン(IOVS, 2011)、PDE阻害剤シロスタゾール(IOVS, 2013)などの薬剤が網膜細動脈を拡張させることを報告し、これら薬剤の網膜血流改善作用が、糖尿病網膜症の発症・進展予防に役立つ可能性を示してきた。
     本講演では、糖尿病網膜症に影響を与える全身因子について、特に同じ糖尿病細小血管症である腎症との関わりについて、これまでの我々の施設で得られた研究データも加えて概説し、糖尿病網膜症による失明予防のための眼科と内科の連携の重要性について考えてみたい。

    平成22年度の研究により、以下の成果を得た。

    1)ネコを用いたin vivo実験により、急性高血糖負荷により、血管内皮依存性の網膜血管拡張因子であるブラジキニンによる網膜血流増加反応が減弱することから、高血糖負荷により網膜血管内皮機能が減弱することが明らかとなった。また、高血糖負荷後に正常血糖に戻すとこのブラジキニンによる血流増加反応が改善することから、適切な血糖改善により血管内皮機能障害が改善されると推測された。さらに、活性酸素除去剤Tempolの前投与によりブラジキニンの血流増加反応が改善することから、この高血糖による血管内皮機能障害には酸化ストレス増加が関与していると推測された。

    2)2型糖尿病における網膜循環動態についてはこれまで詳しい検討がなされていなかった。我々は、網膜症のない、あるいはごく早期の網膜症を有する2型糖尿病患者を対象として、レーザードップラー眼底血流計による網膜血流量測定をおこなった。この横断研究により、網膜症のない病期ですでに網膜血流量は低下しており、軽度網膜症においても血流量は低下したままであることを明らかにした。また、網膜血流量と血中LDLDおよびクレアチニンの値は負の相関関係があり、脂質異常症および腎機能が網膜循環に影響を及ぼすと考えられた。また、網膜血流量が低値である群では、有意にHbAlcが高値であり、血糖コントロール不良により網膜循環が障害されている可能性が示唆された。

    以上の結果から、網膜症早期より網膜循環を改善させる薬剤により適切に介入することによって、その後の糖尿病網膜症の発症・進展を予防できる可能性が示唆された、平成23年度の研究により、新しい糖尿病網膜症治療法の開発につなげたいと考えている。

    平成16年度に以下の研究を行い、その成果を発表することができた。

    1.ネコ眼を用いた実験

    全身麻酔下でネコの前房に針を刺入・留置し、眼灌流液のボトルを上下させることで眼圧を変動させた際の網膜循環動態について検討した。眼圧を40-70mmHgまで10mmHgに段階的に増加させ、眼球加圧負荷とした。負荷はそれぞれ5分間行った。すると、眼圧依存性に負荷終了後に一過性に網膜血流が増加した。また、眼圧60,70mmHgでは負荷終了後の網膜血流増加の際に網膜血管が拡張することも確認した。これは平成17年5月の国際学会で発表予定である。今後は、一酸化窒素、アデノシン、カリウムチャネルなどの阻害剤を用いてこれらの物質の関与を検討したい。

    2.正常人を対象とした実験

    血管内皮機能を評価することは非常に重要であるが、網膜のような細動脈レベルでの評価法は確立されていない。眼球を軽度加圧して眼圧を上昇させたあと眼血流が増加することが知られており、これを利用して新しい網膜血管内皮機能評価を試みた。この研究では、網膜血流の増加を感知して網膜細動脈が拡張することが確認され、網膜血管内皮機能評価が可能であると考えられた。さらに、新たな血管評価法として眼球加熱による眼循環反応の評価法も確立した(論文3)。

    3.患者を対象とした網膜循環動態の研究

    レーザードップラー法を用いて糖尿病患者の眼循環動態を評価すると、網膜症発症前あるいは発症早期から中心窩網膜血流と網膜血流が低下していることを報告した(論文1)。さらに、透析患者では全身循環の変動に網膜循環が大きく影響を受けること(論文2)、さらに虚血性心疾患患者では全身の動脈硬化の指標であるIntima-Media Thickness(IMT)と網膜循環パラメータが相関し、網膜細動脈レベルでの動脈硬化の評価が可能であることも明らかにした(論文4)。

    今後も研究を継続し、糖尿病患者における眼循環動態および網膜血管内皮機能異常の病態把握と解明を行う予定である。

    3)糖尿病患者では視機能に重要な影響を与える黄斑部の脈絡膜血流が網膜症発症前からすでに低下し、黄斑浮腫を生じるとさらに低下することを明らかにした。これより現在糖尿病患者の視力障害の主因である黄斑浮腫に循環障害が関与している可能性が示唆された(論文2)。

    4)健常人を対象に、眼球を加熱したさいの眼循環動態について検討し、眼球加熱により網膜血流は増加するが、黄斑部の脈絡膜血流は反対に減少することを明らかにした(論文3)。
