    Purpose: We aimed to clarify differences in disaster support measures for children with medical complexities (CMCs) and their families in depopulated areas and highlight the need for continuous support in pediatric nursing from local governments.
    Methods: From November 2021 to February 2022, municipal disaster prevention officials in 650 depopulated areas across northern, eastern, and western Japan completed questionnaires. Data from 296 respondents were analyzed.
    Results: By our estimate, over approximately 589 CMCs live in depopulated areas. Depopulated areas in western Japan had an approximately 10 % higher disaster experience rate than general depopulated areas. Despite municipalities recognizing the severity of the CMCs' needs, the evacuation support plan development and evacuation drill performance rates for them remained low.
    Conclusion: Disaster preparedness in depopulated areas requires a robust network of municipalities and medical institutions that support CMCs and their families. Pediatric nurses must implement standardized, evidence-based disaster protocols to ensure the safety of CMCs and their families.

    The aim of this paper is to investigate the potential of virtual reality images as a means of reducing pain and fear in children, through the use of projection technology. While body-worn immersive displays have been previously suggested for this purpose, head-mounted displays have been found to be unsuitable due to their lack of interactivity. To address this issue, we propose a simple and cost-effective two-screen projection system that can be installed in a hospital room without interfering with communication and without compromising the immersive experience. By projecting onto screens installed on the wall and floor, an immersive digital space can be created visually. Our experiments, in which we measured α -amylase activity in saliva, showed that the proposed two-screen projection significantly reduced stress levels in healthy adults when compared to one-screen projection. Additionally, we conducted an experiment using the KOKORO scale as a subjective measure, which revealed that children with medical complexity experienced a significant reduction in stress levels after viewing the two-screen projection compared to before viewing. These findings suggest that the proposed system could serve as an ideal distraction in pediatric nursing, thereby providing much-needed nursing support to children and their families.

    Aim: To develop and validate the Distracting Ingenuity Promotion Scale (DIPS) for paediatric nurses.
    Design: Cross-sectional study using anonymized self-administered questionnaires.
    Methods: From July-November 2013, paediatric nurses working in the wards or outpatient departments in 39 medical institutions in Japan were enrolled in the survey. Data were analysed using the Student's t-test, Mann-Whitney U test and analysis of variance test. Exploratory and confirmatory factor analyses were performed to validate the factors in the DIPS. Cronbach's α was used to calculate the reliability of the DIPS.
    Results: The DIPS included five subscales comprising 21 items. The goodness-of-fit indices for confirmatory factor analysis had a Comparative Fit Index of 0.923 and a Root Mean Square Error of Approximation of 0.059 and fulfilled the standard external validity criteria. Cronbach's α was 0.707-0.826 for each subscale and 0.895 for the overall scale.

    Background and Objectives:
    Nursing care sensitive to children's rights is expected to provide appropriate support in health care settings, which is an integral part of a child-centered approach to care. The purpose of this study is to identify the perceptions of school-aged children with a focus on the United Nations Convention on the Rights of the Child (UNCRC) and children's decision-making processes regarding health care to enhance pediatric nursing practice that takes children's rights into account.

    Methods:
    This cross-sectional study used anonymized self-administered questionnaires among school-aged children attending elementary schools in the city from September to November 2015. Data were analyzed using Pearson's chi-square test, Bonferroni's test, and the Phiφ coefficient. Logistic regression analysis with forced imputation and the likelihood ratio of increasing variables was performed to examine factors that influence application of the UNCRC.

    Results:
    School-aged children who were aware of the UNCRC were likely to be 11 years old (p < 0.05, φ= 0.206) and to have been hospitalized (p < 0.01, φ= 0.220). The factor affecting knowledge of the UNCRC was hospitalization (B = 2.757, odds ratio 15.750, 95% confidence Interval: 1.978, 125.396, p < 0.01). The goodness of fit of the regression equation was the Hosmer and Lemeshow test (p = 0.442) and the discriminant accuracy was 94.9%, indicating a high prediction accuracy.

    Conclusions:
    This study identified information useful to the UNCRC and the child's decision-making process regarding medical care to enhance pediatric nursing practice that considers children's rights. These are important for all pediatric nurses to promote nursing practice that respects children's rights, and to examine the efforts necessary to help medical staff and families decide what is best for the pediatric patient.

The goodness-of-fit indices for confirmatory factor analysis had a Comparative Fit Index of 0.923, a Root Mean Square Error of Approximation of 0.059 and fulfilled the standard external validity criteria. Cronbach's α was 0.707–0.826 for each subscale and 0.895 for the overall scale. [Discussion/Implications] The DIPS, which was developed and validated by the author, showed that paediatric nurses can promote distracting ingenuity in paediatric patients and enable nursing practices to support paediatric patients and their families. Moreover, the DIPS proved to be an essential tool for guiding the resolution of ethical problems for school-aged paediatric patients who experienced psychological outcomes, such as trait anxiety, pain, and fear. This unique tool adds distracting ingenuity, which will be promoted by all pediatric nurses, and new implementable ways to provide support for the psychological outcomes of pediatric patients and their families. However, the reliability and validity of the questionnaire should be further verified in other prospective and controlled studies.

    1.本研究の目的は、過疎地域市町村で医療的ケア児を支援するための災害時システムの構築に向けた、現状と課題を明らかにすることである。そこで当該年度の調査においては、全国の過疎地域市町村における防災対策や医療的ケア児に関する支援の現状把握を行うため「過疎地域市町で医療的ケア児を支援するための災害時システムの構築に必要な全国アンケート調査」を実施した。2.当該年度の調査では、総務省が2021年4月1日現在で発表した「過疎地域の持続的発展の支援に関する特別措置法による過疎地域」の全部過疎に該当する650か所の市町村役場防災担当者および保健福祉担当部署宛てにアンケート用紙を送付し、研究協力の承諾後、質問紙に必要事項を記入の上、299名から回答を得た(回収率46.0%)。結果から、災害を経験していない過疎地域市町村は、災害経験のある過疎地域市町村より避難訓練の実施率が低かったこと、医療的ケア児の避難に関する個別計画の作成が進んでいない現状などを把握することができた。その他の詳細な結果分析については、現在作業を進めている。
