    Anorectal malignant melanoma (AMM) is a rare disease with a poor prognosis, accounting for < 1.0% of all malignant melanomas and a 5-year survival rate of 19.2%. The treatment is mainly surgical, and lymph-node dissection is often performed. Cases of recurrence after a prolonged period (> 10 years), as in the present case, are rare. The patient was an 80-year-old woman who underwent laparoscopic intersphincteric resection with bilateral lateral lymph-node dissection for the diagnosis of primary AMM of the lower rectum at X - 12 years. The pathology was pStage III and the resection margins were negative. Twelve years after the initial surgery, in year X, the patient visited our hospital with the chief complaint of discomfort due to a tumor in the anorectal region. Biopsy revealed a recurrence, and laparoscopic abdominoperineal resection was performed. Based on the pathological findings, the patient was diagnosed with local recurrence of the anastomotic anal canal. Both the first and second specimens were negative for BRAF V600E/K mutation. Four months have passed since the surgery, and the patient continued to receive nivolumab without recurrence. Long-term local follow-up is necessary when the anal canal is preserved during AMM surgery.

    BACKGROUND: Preservation of plantar subcutaneous fat is crucial for cushioning in the surgical treatment of acral melanoma of the sole. However, no studies exist on the relationship between deep margins and prognosis. We aimed to retrospectively compare the prognoses of different deep margins (within or beyond the subcutaneous fat) in patients with invasive acral melanoma of the sole who underwent wide local excision. METHODS: In this multi-institutional retrospective study, survival was compared between 2 groups of patients: those with tumors excised within (S group) and those beyond the subcutaneous fat (D group). RESULTS: In total, 464 patients were included. Cox multivariable analyses showed that the depth of the deep excision margin was not associated with local recurrence-free survival, overall survival, or distant metastasis-free survival (hazard ratios of 1.20, P = .36; 1.10, P = .66; and 1.42, P = .05, respectively). However, excision beyond the subcutaneous fat was negatively associated with disease-free survival (hazard ratio 1.45, P = .02). After propensity score matching (both groups, n = 139), no significant differences were observed in survival outcomes between the S and D groups (5-year local recurrence-free survival: 72.8 vs 66.8%, P = .55; 5-year disease-free survival: 55.3 vs 43.7%, P = .24; 5-year overall survival: 76.2 vs 73.2%, P = .52; 5-year distant metastasis-free survival: 63.3 vs 54.1%, P = .13). Subgroup analysis of American Joint Committee on Cancer stages revealed no significant differences in survival outcomes between the 2 groups at any stage. CONCLUSION: Wide local excision beyond the subcutaneous fat was not associated with survival benefit of acral melanoma of the sole. Excision within the subcutaneous fat may represent the optimal deep margin.

    PURPOSE: Adjuvant anti-PD-1 (adj PD-1) antibodies are extensively used to improve survival in patients with resected melanoma. Clinical trials on adj PD-1 antibodies have revealed significant improvements in recurrence-free survival (RFS); however, few of these trials have included patients with acral melanoma (AM). METHODS: Clinical data were retrospectively collected from Japanese patients who underwent resection of stage III sole AM between 2014 and 2021. Survival outcomes, including RFS, distant metastasis-free survival (DMFS), and overall survival (OS), were compared between patients without adjuvant therapy (OBS group) and those receiving adj PD-1 group. RESULTS: This study included 139 patients (OBS: 79; adj PD-1: 60), with a median follow-up of 2.6 years. The baseline characteristics were comparable, except for age and nodal metastasis. No significant differences in survival were observed between the OBS and adj PD-1 groups (3-year RFS: 36.7% v 27.5%, P = .13; 3-year DMFS: 51.0% v 45.3%, P = .51; 3-year OS: 65.3% v 67.4%, P = .45). Multivariate analysis showed no survival benefit of adj PD-1 (RFS: hazard ratio [HR], 1.25, P = .29; DMFS: HR, 1.03, P = .89; and OS: HR, 0.69, P = .23). Each survival outcome after propensity score matching confirmed no significant difference between the matched OBS group (n = 52) and adj PD-1 group (n = 52; 3-year RFS: 34.3% v 25.9%, P = .22; 3-year DMFS: 45.6% v 46.5%, P = .85; 3-year OS: 60.7% v 68.9%, P = .29). CONCLUSION: Adj PD-1 did not improve the prognosis in sole AM. However, further studies are essential to evaluate the efficacy of the adj anti-PD-1 antibody in AM.

    The diagnosis of systemic amyloidosis relies on the detection of amyloid deposition in the tissue, often utilizing biopsy specimens from the abdominal skin owing to their minimal invasiveness. Several amyloid staining methods, including Congo Red, Direct Fast Scarlet (DFS), and Thioflavin T (ThT), have been employed for visualization. Lipomembranous fat necrosis (LFN) is a non-specific reaction pattern of adipose tissue to injury, typically derived from blood insufficiency across a wide range of clinical conditions or diseases. It is characterized by the presence of eosinophilic, crenulated, and/or serpiginous membranes in fat lobules. We encountered a patient in whom ThT yielded suspiciously positive results in amyloidosis screening tests. Furthermore, our retrospective observations suggested that ThT staining was positive for LFN, whereas DFS and Congo red staining yielded negative results. The awareness that LFN can result in false-positive ThT staining during amyloid screening is crucial to avoiding the misdiagnosis of systemic amyloidosis. Furthermore, skin samples should not be collected from areas prone to developing lipomembranous changes. The use of more than two different stains for skin biopsy specimens is recommended.

    Whether sentinel lymph node biopsy status is a prognostic factor or effective in determining treatment strategies in extramammary Paget disease remains unclear. This study aimed to investigate the significance of sentinel lymph node biopsy in extramammary Paget disease. We retrospectively reviewed the clinical information of previously untreated patients with invasive extramammary Paget disease who underwent wide local excision of the primary tumor and sentinel lymph node biopsy at our hospital between April 2008 and March 2021. Clinical data including the baseline neutrophil-to-lymphocyte ratio and recurrence-free survival were analyzed. Sentinel lymph node metastases were classified as macrometastases and micrometastases, with a cut-off value for sentinel lymph node tumor burden of 2 mm. Univariate and multivariate analyses of factors affecting sentinel lymph node biopsy positivity and recurrence-free survival rates were performed. Overall, 85 patients were included in the analysis. Patients in the sentinel lymph node biopsy-positive group (n = 26) had a significantly higher invasion level and neutrophil-to-lymphocyte ratio. According to multivariate analyses, invasion level and a high neutrophil-to-lymphocyte ratio were independent predictive factors for sentinel lymph node biopsy positivity, and the sentinel lymph node biopsy status was an independent prognostic factor for recurrence-free survival. In conclusion, sentinel lymph node biopsy provides an accurate risk classification and clinical indication for postoperative follow-up in patients with invasive extramammary Paget disease.

    The neutrophil-to-lymphocyte ratio (NLR) is a useful prognostic biomarker for many cancer types. However, the prognostic value of NLR in patients with extramammary Paget disease (EMPD) remains unclear. This study aimed to determine whether NLR is associated with overall survival (OS) in patients with EMPD. In this single-center retrospective case series, we analyzed data of 109 patients with previously untreated EMPD who presented to our hospital. Data on age, sex, primary tumor site, invasion level, presence of lymph node and distant metastases, baseline NLR, and OS were analyzed. The enrolled patients were classified into the metastatic or non-metastatic EMPD groups. The metastatic EMPD group had higher invasion level and NLR value (p < 0.001 and p = 0.005, respectively) compared with the non-metastatic EMPD group. Cox proportional hazards model analysis showed invasion level (p = 0.0093) and NLR value (p = 0.019) to be independent prognostic factors for OS. Notably, multivariate analysis revealed that invasion level (p = 0.045) and NLR value (p = 0.036) were independent prognostic factors for OS in the metastatic EMPD group. The limitations of this study include the small number of participants and its retrospective nature. In conclusion, since NLR can be routinely assessed with high feasibility, it might be a useful biomarker for predicting prognosis in patients with EMPD.

    BACKGROUND: The neutrophil-to-lymphocyte ratio (NLR) is a known prognostic biomarker for survival and is predictive of sentinel lymph node (SLN) positivity in some cancers. However, its usefulness as a prognostic biomarker for cutaneous squamous cell carcinoma (cSCC) has not been fully investigated. OBJECTIVE: Our objective was to investigate the relationship between the NLR and the disease-specific survival and SLN positivity in patients with cSCC. METHODS: In this single-center retrospective case series, we analyzed patients with cSCC who underwent blood tests prior to the initiation of treatment at our oncology hospital. The relationship between the patients' clinical characteristics (including the NLR) and the disease-specific survival and SLN positivity was evaluated using univariate and multivariate analyses. RESULTS: An elevated NLR was an independent prognostic factor for poor disease-specific survival and a predictive factor for SLN positivity. LIMITATIONS: Limitations include the small number of participants and selection bias due to the large proportion of high-risk cases in our patient population. CONCLUSION: NLR is a useful biomarker in cSCC because it is simple to measure and can predict prognosis.
