ObjectiveTo investigate the relationship between chronic periodontal disease (CPD) and lower urinary tract symptoms (LUTS) in both sexes.
MethodsThe interview sheet of the CPD self-checklist and LUTS was distributed to 600 adult men and women (300 each) who visited the first dental examination at dental clinics. The International Prostate Symptom Score (IPSS) questionnaire/Quality Of Life (IPSS/QOL) and Overactive Bladder Symptom Score (OABSS) were used to assess LUTS. The relationship between the CPD score and LUTS or OAB was examined.
ResultsThe interview sheet was collected from 88 men (50.916.6years old) and 97 women (51.1 +/- 15.5years old). There was no statistically significant correlation between the CPD score and age, or between the CPD score and the presence of LUTS in either men or women. However, urgency and weak stream score of IPSS were significantly correlated with the severity of CPD in both sexes. Significant correlation between the severity of CPD and the presence of OAB was only noted in men but not in women.
Conclusions<p id="luts12042-para-0004">The present study demonstrated for the first time that some storage and voiding symptoms were significantly associated with CPD in both sexes. Thus, although CPD and LUTS seem to have common pathophysiological factors, the interrelationship between CPD and LUTS is slightly different between men and women.

S-1 chemotherapy is widely used to treat head and neck cancer. Reports of interstitial pneumonia as a side effect of S-1 chemotherapy are very rare.<br>We report a case of interstitial pneumonia possibly caused by S-1 treatment as postoperative adjuvant chemotherapy.<br>We gave S-1 as postoperative adjuvant chemotherapy for mandibular gingival carcinoma in a 80-year-old woman. She had a fever 53 days after starting S-1 chemotherapy. The X-ray films and computed tomography （CT） showed reticular shadows in both lung fields, and the patient was given a diagnosis of interstitial pneumonia. The results of a drug-induced lymphocyte stimulation test were positive against S-1. The total dose of S-1 until the onset of symptoms was 3360 mg. We immediately started steroid therapy and withdrew S-1 chemotherapy, and then the symptoms remarkably improved, as did the abnormal findings on CT.<br>In conclusion, when a patient has a high fever and dyspnea during S-1 therapy, drug-induced interstitial pneumonia should be included in the differential diagnosis.

We devised a treatment protocol for simple tooth extraction that conforms to the "Guidelines for management of anticoagulant and antiplatelet therapy in cardiovascular disease (JCS2004)". Using this protocol, 1,020 simple dental extractions were performed in patients on antithrombotic therapy (patient group) and in control patients (control group).<br>We statistically analyzed bleeding after tooth extraction following complete hemostasis (postoperative bleeding), difficulty stopping bleeding after tooth extraction (hemostatic difficulty), background factors, contents of hemostasis, antithrombotic medications, oral surgery specialist qualification of the operator, and PT-INR.In the patient group, 56 cases with hemostatic difficulty were observed (14.7%) and postoperative bleeding (3.1%) after extraction occurred in 12 of the 382 cases. In the control group, there were 12 cases with hemostatic difficulty (1.9%) and one with postoperative bleeding (0.2%) among the 638 undergoing extraction. Hemostatic difficulty and postoperative bleeding requiring serious systemic hemostatic intervention was not observed in any of the cases undergoing extraction. Factors related to hemostasis difficulties were local inflammatory and anti-platelet therapy and warfarin therapy. Factors associated with postoperative bleeding were oral surgery specialist qualification and warfarin therapy.<br>In this study, tooth extraction was performed employing our protocol which is based on compressive hemostasis. Tooth extraction can be safely carried out during antithrombotic therapy with appropriate local hemostasis. Furthermore, hemostasis was possible with a local hemostatic method in all cases of the patient group and the control group.

To examine the relationship between chronic periodontal disease (CPD) and ED, the interview sheet including the CPD self-checklist (CPD score) and the five-item version of the International Index of Erectile Function (IIEF-5) was distributed to 300 adult men who received a comprehensive dental examination. Statistical analyses were performed by the Spearman's rank correlation coefficient and other methods. Statistical significance was accepted at the level of P&lt; 0.05. The interview sheets were collected from 88 men (response rate 29.3%, 50.9 +/- 16.6 years old). There was a statistically significant correlation between the CPD score and the presence of ED (P = 0.0415). The results in the present study suggest that ED is related to the damage caused by endothelial dysfunction and the systematic inflammatory changes associated with CPD. The present study also suggests that dental health is important as a preventive medicine for ED.

This report presents a case of chronic mandibular dislocation treated by conservative reduction in a patient who was receiving anticoagulant therapy. The patient was a 65-year-old woman who was referred to the Department of Oral and Maxillofacial Surgery in Asahikawa Medical University for the treatment of chronic mandibular dislocation. Chronic mandibular dislocation had developed during hospitalization for cardiogenic embolism. Since the patients physician recommended us to continue anticoagulation therapy, we decided to perform conservative reduction by lever action, using intraoral blocks with built-up heels and elastic traction. Because the patient was edentulous, we innovated on past reports of conservative reduction by lever action using teeth. Eleven days after treatment, the reduction was successful, and the jaws were stable. By 1 year after treatment, there was no recurrent dislocation, and the outcome was good.

Methotrexate （MTX） is administered to patients with rheumatoid arthritis （RA） as standard treatment. Tumor necrosis factor （TNF） antagonist, which was recently approved for use in Japan, also shows excellent clinical effectiveness. However, these agents may increase the risk of malignant lymphoma. We report a case of methotrexate-associated lymphoproliferative disorder （MTX-LPD） after chemotherapy with MTX and TNF antagonists. <br>A 52-year-old woman presented because of pain in the left oral floor with an ulcer in June 2009. The patient had received MTX therapy for RA for 3 years since 2006, as well as TNF antagonists for 2 years from 2007. The diagnosis of' diffuse large B-cell lymphoma' was established by biopsy of the focus. MTX therapy was discontinued on consultation with her doctor, and the ulcer disappeared in 4 months without any further chemotherapy for lymphoma.

Oral myxoma usually arises in the jawbone and rarely occurs in soft tissue. A case of myxoma in the gingiva of the upper jaw is reported. The patient was a 75-year-old man, who had noticed a tumor in the right molar gingiva 40 years ago. The tumor became larger after extraction of an upper molar. A mushroom-shaped elastic hard tumor (20 × 20 × 15 mm) was noted in the upper molar gingiva. Radiographic and computed tomographic examinations showed a tumor in the maxilla, but extensive bone resorption was not observed. An incisional biopsy was done, and the diagnosis was myxoma. Partial maxillectomy including the tumor was performed with the patient under general anesthesia. Histopathological examination showed that the tumor was composed of spindle-shaped cells in a myxoid matrix. The tumor was situated mainly in the gingiva, but slight pressure-related bone resorption was observed on the surface of the alveolar bone. Four years after treatment, there was no evidence of recurrence.

Langerhans' cell histiocytosis is an uncommon disease characterized by proliferation of Langerhans' cells. We describe a 24-year-old man with Langerhans' cell histiocytosis in the right side of the mandible. The lesion was present only in the right mandibular bone and was removed by surgical curettage. The patient had an uneventful recovery. There is no evidence of recurrence as of 3 years 6 months of follow-up.

We clinicostatistically studied 102 cases of external dental fistula at the Department of Oral and Maxillofacial Surgery, Asahikawa Medical College during the past 18 years (from 1977 through 1994). The following results were obtained:<BR>1) The 102 patients comprised 49 males and 53 females. As for age, patients, in their twenties and thirties were most frequently encountered.<BR>2) The period from the onset of disease until the first visit at our clinic tended to be relatively long. The longest period was 13 years and the shortest several days. The average period was 16.9 months.<BR>3) The cheek area (48.0%) was the commonest site of external dental fistulae.<BR>4) External dental fistula arose more often in the mandibular region than the maxillary region, and was especially frequent in the mandibular molars and incisors.<BR>5) In many patients the involved teeth and the fistula were concurrently treated by surgery.

Twenty-eight patients with carcinoma of the tongue treated at the Department of Oral and Maxillofacial Surgery, Asahikawa Medical College from 1978 to 1986 were reviewed.<BR>The patients ranged in age from 33 to 80 years, and 71. 4 percent were male (20/28). The carcinoma in 26 cases originated in the lateral borders of the tongue. The histopathological diagnosis was squamous cell carcinoma in all cases. In stage grouping of the UICC (1978): 11 cases belonged to stageI, 8 to II, 5 to III, 4 to IV.<BR>In early cases of stage I and II, 8 cases were treated mainly by interstitial radiotherapy and 11 cases mainly by surgery. In advanced cases of stage 111 and IV, most of them received surgical treatment combined with pre-operative irradiation. The 5-year survival rate of stage I, II, III, IVwere78.9%, 85.8%, 60%and50%, respectively.<BR>In cases of stage 1 and 1I, the cure rate treated by surgery was clearly superior to that by radiation therapy.<BR>Cases with metastasis to cervical lymph nodes showed. markedly lower survival rate than those without cervical metastasis. But no case with metastasis to cervical lymph nodes treated by neck dissection died from cervical metastasis. Recurrences of primary lesions or cervical metastasis mostly occurred within 1 year after the treatment.<BR>This indicates that we must regularly followup the subsequent history of the patients within 1 year after the treatment.

We studied the effect of bone substitutes on revascularization and the restart of blood supply after sintered bone implantation in comparison with synthetic hydroxyapatite implantation and fresh autogenous bone transplantation (control) in rat parietal bones. Methods for the study included the microvascular corrosion cast method and immunohistochemical techniques were also used.
The revascularization of the control group was the same as that for usual wound healing in the observations of the microvascular corrosion casts. The sintered bone implantation group was quite similar to that of the control group. In the synthetic hydroxyapatite group, immature newly-formed blood vessels existed even on the 21st day after implantation and the physiological process of angiogenesis was interrupted.
Immunohistochemically, vascular endothelial growth factor (VEGF), which activates angiogenesis, appeared at the early stages of both the control group and the sintered bone implantation group. VEGF reduced parallel with the appearance of the transforming growth factor factor-beta-1 (TGF-beta-1), which obstructs angiogenesis, and the angiogenesis passed gradually into the mature stage. In the hydroxyapatite implantation group, TGF-beta-1 appeared at the early stage of the implants. The appearance of VEGF lagged and it existed around the pores of hydroxyapatite even on the 21st day of the implantation. Proliferation and wandering of endothelial cells continued without any maturing of the vessels.
These findings suggest that the structure and the components of the implant material affect angiogenesis after implantation as well as new bone formation.

Two cases of sarcoma in the temporomandibular joint region are presented - an osteosarcoma and a chondrosarcoma. These tumours in the maxillofacial region, particularly in the temporomandibular joint, are very rare. The patients underwent surgical treatment alone. The follow-up periods were 9 and 6 years, respectively, and no local recurrence or distant metastases were encountered. © Elsevier Science Ltd. All rights reserved.

Giant cell tumor most commonly occurs in the epiphysis of long bones. This tumor is extremely rare in the maxillofacial region.<BR>We describe a case of giant cell tumor of the maxilla. The patient was a 33-year-old woman with a diffuse swelling on the left side of her face and a mass on the left alveolar bone of the maxilla. An orthopantomogram showed a round soft tissue shadow with an area of osteolysis and resorption of the roots of the premolars in the left side of the maxilla. Biopsy revealed a giant cell tumor, and a left partial maxillectomy was performed intraorally. The final diagnosis was giant cell tumor. No tumor recurrence has occurred during 2 years of follow-up.

To determine the appropriate time for bone reconstruction after irradiation, the healing process after autogenous iliac bone transplantation in the irradiated parietal bone was examined by scanning electron microscopy and light microscopy. Bone transplantation was carried out at the second and the fourth weeks after Cobalt-sixty (Co-60) irradiation with calculated dose and fractionation. Animals without irradiation were used as control.
The results show the appearance of mesenchymal cells and blood vessels around the transplantation to be extremely few one week after transplantation which was carried out at the second week after irradiation. These inhibitions were still seen two weeks after transplantation. Four weeks after transplantation, there were no differences in the bone formation among the experimental groups. Bone formation in the transplantation at the fourth week after irradiation was similar to that of the control group. Microvascularization in the transplantation at the second week after irradiation was inhibited one week after transplantation.
The delay in bone healing was responsible for the retardation of revascularization and caused microcirculatory failures as well as the damage of osteogenic cells. It is quite clear that damaged cells and tissues recovered by the elapse of time under the irradiation procedure employed in this study and also that bone formation was carried out in the physiological process. We think that bone transplantation after irradiation should be done after recovery from the radiation damage to the periosteal cells and the blood vessels.

A 49-year-old woman was referred to our department because of a swelling at the left mandibular angle. Extended radical mastectomy had been performed 6 months previously, and the histopathological diagnosis was papillotubular carcinoma. One month after the mastectomy, the patient experienced discomfort of the left mandibular angle and hypalgesia of the left side of the lower lip. Intraoral examination revealed a hard mass, measuling 6×4 cm and covered normal mucosa, in the left lower molar region. Occlusal radiography showed a sun-ray-like appearance at the focus. Bone scintigraphic scans showed an area of increased intake of radiolabeled technetium in the left side of the mandible. Preoperative chemotherapy with a high dose of methotrexate had been given in accordance with Rosen's T-12 protocol, but the tumor grew rapidly. Radical resection of the tumor was performed. Postoperatively, she was given chemotherapy with CDDP and ADM. There have been no clinical or radiological signs of recurrence or distant metastasis as of 9 years after operation.

The healing process of resected, autoclaved (121 degrees C, 20 minutes) and re-implanted bone in the rat parietal bone was compared with that of autoclaved bone that was supplemented with allogenic bone matrix (AAA-bone), using a scanning electron microscope and a light microscope.
In the implant without AAA-bone, bone union and replacement of the autoclaved bone was seen at 2 weeks after implantation. There was no evidence of any inflammatory reaction around the autoclaved bone. The implant was gradually replaced by the new bone. In the implant with AAA-bone, the new bone formation around the implanted bone was more abundant than that of the implant without AAA-bone. An inflammatory reaction was also observed after 1 week. The replacement of the implant with AAA-bone was inferior to the nonsupplemented group. The reason for the poor replacement was the disturbance of the blood supply in the implant by abundant new bone formation.
In these results, the autoclaved bone re-implantation was an excellent bone substitute with osteoconductive ability and biocompatibility. The implantation with AAA-bone was good for the new bone formation, but the position and the technique of supplement with AAA-bone have to be more deeply investigated.

Conventionally, an ameloblastoma with metastasis was diagnosed as a malignant ameloblastoma. In the new classification of odontogenic tumors revised by the WHO in 1992, the diagnosis of malignant ameloblastoma is based on cytological criteria. Malignant ameloblastoma is defined as a neoplasm in which the pattern of an ameloblastoma and the cytological features of malignancy are shown by the primary lesion in the jaws and/or any metastatic lesion.<BR>We reported a case of recurrent ameloblastoma of the right mandibule with metastatic lymph nodes six years after primary treatment. There was metastasis to the buccal and submandibular lymph nodes on the right side. There were no cytological features of malignancy in either the mandibular lesion or metastatic lymph nodes.<BR>According to general concepts of pathology, the criteria for malignancy includes recurrence and metastasis as well as cytological features. The criteria for malignant ameloblastoma require further discussion.

Malignant ameloblastoma is defined as a neoplasm in which the pattern of an ameloblastoma and the cytological features of malignancy are shown by the primary lesion in the jaws and/or any metastatic lesion.<BR>We reported a case of recurrent ameloblastoma of the right mandibule with metastatic lymph nodes six years after primary treatment. There was metastasis to the buccal and submandibular lymph nodes on the right side. There were no cytological features of malignancy in either the mandibular lesion or metastatic lymph nodes.<BR>According to general concepts of pathology, the criteria for malignancy includes recurrence and metastasis as well as cytological features. The criteria for malignant ameloblastoma require further discussion.

The healing process after implantation of sintered bone in the rat parietal bone was compared with that of synthetic hydroxyapatite using both scanning electron and light microscopy. The results showed that the differences between the sintered natural bone and the synthetic hydroxyapatite implantations were in the states of bone union and the bioresorbability of the implanted materials, even though both materials consist of the same hydroxyapatite. In the sintered bone implantation, the newly formed bone invaded into the material at 1 to 2 weeks after implantation. The sintered bone surface on the dura mater side was completely covered by the new bone at 5 weeks. It is noteworthy that bone resorbing areas characterized by Howship's lacunae were observed on the sintered bone surface at 2 weeks and the material was replaced by new bone. Light microscopy, which revealed the invasion and the development of the new bone into the sintered bone, supported the scanning electron microscopic observations. In the synthetic hydroxyapatite, the new bone adhered closely to the material just like the sintered bone implantation. The new bone did not invade into the synthetic hydroxyapatite. There was no evidence of the resorption of the hydroxyapatite. This shows that the natural and the biological structures of the sintered bone offer an advantageous environment to fluid circulation and ingrowth after implantation.

Clinicostatistical investigation was made on about 53 patients with squamous cell carcinoma of the oral cavity who had undergone neck dissection at the Department of Oral and Maxillofacial Surgery, Asahikawa NIcdical College from January 1979 to December 1988.<BR>Of the 71 cases of neck dissection, 50 were performed therapeutically, and 21 prophylactically. Metastases to the cervical lymph nodes were histologically confirmed in 33 patients.<BR>The majority of the involved nodes were submandibular lymph nodes, superior internal jugular nodes, and mid internal jugular nodes.<BR>The five-year-survival rate was 84.1% in patients without metastasis and 46.4% with metastasis.<BR>Prognosis was poorer as the number of metastatic nodes and sites increased. Prognosis was poorer as cervical nodes of lower level were involved.<BR>Of the 33 cases with cervical lymph node metastasis, eleven cases died of local failure, but only one case died of failure in the neck. This indicates that the prognosis of cases with cervical lymph node metastasis depends upon whether the primary disease is controlled or not.

The dentition of front and molar part teeth showed a state of normal occlusion and the maxillo-mandibular were mainteined in good condition.<BR>Our results suggested that the maxillar growth capacity in cleft palate patients was similar to that of normal adults.

Twenty-eight patients with carcinoma of the tongue treated at the Department of Oral and Maxillofacial Surgery, Asahikawa Medical College from 1978 to 1986 were reviewed.<BR>The patients ranged in age from 33 to 80 years, and 71. 4 percent were male (20/28). The carcinoma in 26 cases originated in the lateral borders of the tongue. The histopathological diagnosis was squamous cell carcinoma in all cases. In stage grouping of the UICC (1978): 11 cases belonged to stageI, 8 to II, 5 to III, 4 to IV.<BR>In early cases of stage I and II, 8 cases were treated mainly by interstitial radiotherapy and 11 cases mainly by surgery. In advanced cases of stage 111 and IV, most of them received surgical treatment combined with pre-operative irradiation. The 5-year survival rate of stage I, II, III, IVwere78.9%, 85.8%, 60%and50%, respectively.<BR>In cases of stage 1 and 1I, the cure rate treated by surgery was clearly superior to that by radiation therapy.<BR>Cases with metastasis to cervical lymph nodes showed. markedly lower survival rate than those without cervical metastasis. But no case with metastasis to cervical lymph nodes treated by neck dissection died from cervical metastasis. Recurrences of primary lesions or cervical metastasis mostly occurred within 1 year after the treatment.<BR>This indicates that we must regularly followup the subsequent history of the patients within 1 year after the treatment.

Verrucous carcinoma, a variant of squamous cell carcinoma, was first discribed as a distinct clinical and pathological entity by Ackerman in 1948.<BR>In this paper, four cases of verrucous carcinoma of the oral cavity are reported. All except case No.3 could not be diagnosed verrucous carcinoma by the initial biopsies. Especially small superficial biopsies may be often interpreted as benign tumors.<BR>Two cases were treated by surgical excision, the others by cryosurgery as the first choice treatment.<BR>The prognosis of the two surgical excision patients was good, but the others was poor. These results suggested that complete surgical excision obtained satisfactory results in verrucous carcinoma, but this may be precluded by the medical status of the patient or the lesion extent.

The antigenicity of freeze dried and fresh cartilage allografts placed orthotopically in rabbits was studied using a method of <SUP>51</SUP>Cr release assay, and the repair processes in freeze dried and fresh allografts were studied histologically. Freeze dried cartilages were prepared by quick freezing at the following temperatures for three hours and then were dried under a high vacuum. The former process is a preliminary freezing.<BR>Study groups were divided into three groups according to the temperatare of the preliminary freezing. Each groups of I, II, III were-40°C, -60°C and -80°C, respectively. As a control, fresh cartilage allografts were made.<BR>The results may be summarized as follows:<BR>1. Apparently freeze dried cartilage allografts were incorporated by the host, that is, new bone formation, revascularization and bone substitution about freeze dried allografts proceeded. But fresh cartilage allografts were not incorpocated by the host.<BR>2. The difference in rate of revascularization and new bone formation among study groups was not significant.<BR>3. Both freeze dried and fresh cartilage allografts failed to evoke a cell-mediated response measurable by a method of <SUP>51</SUP>Cr release assay.

We have clinico-stastically studied 233 cases of maxillo-facial fracture patients who had been treated during the past eight years from January, 1978 to December, 1985.<BR>From this study, we obtained the following results:<BR>Of all the patients, 185 were male and 48 were female. The male/female ratio was 4: 1. Most of the males were distributed among the ages of 10 to 19 years old (28.3%) and their twenties (26.1%). The number of patients in our department show a tendency to increase every year.<BR>The causes of fractures were mostly traffic accidents (44.1%). One hundred eighty-five cases (79.5%) were referred from public or private hospitals, and only 37 cases (15.9%) from dental clinics.<BR>Fractures of the mandible alone accounted for 69. 1%. The sites of the mandible fractures were found at the condyle (25.3%), the angle of the mandible (21.9%), and other areas (52.5%).<BR>Disturbance of mouth opening were found in 67. 8% of all cases as complications of the fracture, followed by tooth injuries (62.3%), injury of the oral mucosa (36.1%), deformity of occlusion (31.3%) and unconsciousness (24.9%).

They were diagnosed as BCNS because multiple jaw cysts, skeletal anomalies, and many other symptoms accompanied although basal cell nevus was lacked. Cleft lip or cleft palate (bifid uvula) was associated with each case.<BR>Accordingly the ratio of association of cleft lip and/or palate is 15.6%, and this is far more than the ratio in the normal population. 脂肪組織由来幹細胞（ADSC)を用いた骨創治癒の改善についての研究
ラット頭頂骨に骨欠損を形成した後に、ADSCを静脈内投与すると新生骨形成が旺盛となるため骨欠損の治癒が改善される。その機序としては、ADSCが骨形成部に集束し骨芽細胞に分化する直接的作用の関与を示唆した。
