    1群の事前テスト・事後テスト計画を用いた分娩中シミュレーション教育プログラムの効果

    バックグラウンド
    助産師の継続教育は、看護師と連携して計画されることが多いですが、助産の仕事の範囲内で計画する必要があります。
    目標
    本研究の目的は、分娩中の助産能力向上のために開発されたシミュレーション教育プログラムの効果を検討することである。
    メソッド
    助産経験1年以上の助産師21名が参加しました。このプログラムの教育目標は、胎盤分娩後に発生する出血を判断し、適切に対応することを学ぶことでした。学習方法は、7人の参加者からなる1つのグループでシミュレーション学習で構成されていました。メンバー全員が役割(助産師、産科医、妻、夫)を交代し、状況への対応方法を学びました。学習機会は3日間続き、2日間は技術演習、3日間はOSCEで構成されました。毎回報告会が行われました。高価な医療機器は、実際の写真を教材として使用していました。調査項目は、(1)分娩中の助産能力の自己評価点、(2)自己効力感尺度、(3)日本語版自主学習準備尺度、(4)技術点、(5)参加満足度である。本研究は、札幌市立大学大学院看護学研究科倫理審査委員会(第26号)で承認されました。
    業績
    19人の参加者が研究を完了しました。分娩中の助産能力の自己評価スコアは149.8ポイントから164.9ポイント(p < 0.001)に上昇し、日本語版自主学習準備尺度のスコアは199.4ポイントから205.7ポイント(p < 0.05)に上昇した。参加者の満足度(最大5ポイント)は4.6から4.9の範囲でした。
    結論
    助産能力の点数と学習の自己決定能力が有意に増加したため、学習効果の達成が確認された。

    目的:オンラインと対面のハイブリッド演習を受講した学生の技術評価と満足度に基づく教育評価を実施する。
    方法:本研究は定量的研究デザインを採用した。調査期間は2021年4月から5月でした。研究参加者は、A大学実践看護技術II.(母性看護学)3年生の学生61名中41名で、本研究に同意した。評価項目は、採点のための筆記試験の点数と、演習計画のための満足度アンケートであった。本研究は、旭川医科大学倫理委員会(承認番号:20097)で承認されました。
    【結果】筆記試験の平均点は79.5点で、前年度に同じ方法を採用した3学年の平均点(78.8点)と同程度であった。満足度アンケートの学習ニーズについては、全回答者の72.8%が「満足している」と回答した。回答者の80%以上が、このコースが母親の看護への関心を高め、学習上の問題を発見するのに役立ち、自分自身の学習を促進したと回答しました。
    結論:筆記試験の得点は2020年度の平均点と同程度であり、ハイブリッド型演習は経時的に良好と評価された。ハイブリッド型演習は、70%以上の生徒が満足のいく学習方法として評価され、母親の看護への関心、学習課題の発見、自習の促進に資すると考えらた。

    A大学1年課程の助産師養成機関におけるOSCEを助産学実習前後に実施した評価表について前後比較を行い、OSCE実習前後実施の教育的意義と課題を明らかにした。その結果、学生自身が客観的項目とポイントにより形成評価を意識できる。また、就学前が実習後のOSCE実施時期であったことから、就学前準備として自己の助産実践能力の課題が明確化し、継続的に学ぶ動機付けにつながることが予測できた。

    訪問看護師が訪問活動で大切にしていることを、アンケートの自由記述についてコード化、カテゴリー生成することで、病院看護師と訪問看護師の連携に向けた示唆をえることを研究目的とした。その結果、訪問看護師は住み慣れた場所でその人らしい暮らしを支えるために安全で安心な生活を基盤とし、在宅生活継続のための体調維持管理・苦痛をコントロールする医療的介入を行い、利用者と家族を一つの単位として中心に据え、笑顔で穏やかに過ごす時間を大切にしていることが分った。

    We developed a “wearable fetus and uterus model” (Model) for nursing students to practice observations of the uterus of postpartum women, and investigated the learning effects of the Model. The students felt that they were able to use the skills they learned from the reactions of postpartum women paying attention to reactions of comfort and lack of comfort. The students felt that they were able to prepare psychologically before the training, and that they were able to compensate for lack of actual experience after the training. In the Objective Structured Clinical Examination, they provided explanations to and observed postpartum women by showing consideration to the postpartum women. The findings suggest that using the Model will help improve clinical skills in the current basic nursing education where the opportunities and types of skills permitted in the clinical practicum are restricted.

    The aim of this study was to develop an educational program to increase perinatal safety for pregnant women who must travel long distances to give birth. We interviewed such women who live in town A, as well as public health nurses in town A and midwives working at delivery facilities, to determine the educational needs of these pregnant women and their current educational opportunities, and to investigate possible educational programs. We found that the program should include instructions for pregnant women how to prepare themselves during labor and how to call an ambulance in an emergency. while these pregnant women hoped to receive education before delivery, they have few educational opportunities. The primary challenge is to hire professionals with specialized knowledge about delivery.

    In order for midwifery students to acquire the required skills, we developed an educational program using the OSCE, Objective Structured Clinical Examination, as part of the one-year graduate course in midwifery.
    This program is comprised of four OSCE subjects with clearly defined achievement goals. This program also enables us to assess the students' degree of achievement of basic midwifery skills before and after practical training.
    As a result, instructors and students are able to clarify their areas for improvement in future task-based learning and teaching.

    A new evaluation questionnaire survey to measure the work abilities of currently working midwives was prepared. From the results of the survey of midwives in the Hokkaido and the Tohoku region, survey items for which their work ability was low during the perinatal period and at other times were clarified. Low abilities during the perinatal period were consistent with abilities for which midwife students were evaluated as having a low level of attainment. The higher the ability in the perinatal period, the higher the ability at other times. Additionally, the higher the ability in the perinatal period, the higher the level of occupational-preparation-type behaviour and self-determination-type learning. Based on the low-ability items on the new evaluation questionnaire survey, the draft of a postgraduate training and education plan was prepared.

    One study involved pregnant women who lived a long distance away from the hospital or clinic at delivery. The results demonstrate that they felt it was important for information to be given to their friends and family, suggesting the necessity of health education programs including group work and peer support during preparations for delivery. Subjects of another study were nurses and nurse midwives who work in obstetric hospitals and clinics. Nurses and nurse midwives address pregnant women's problems ; pregnant women may have physical, economic, and mental problems and have to coordinate with their family to travel a long distance to the hospital and clinic. In addition, they have medical problems because they may deliver in their house or car without attending medical professionals. The results suggest that the program should include instructions for pregnant women who need a long-distance transfer to hospitals or clinics on how to prepare themselves during labor progression and when to call an ambulance in an emergency.

    The purpose of this study was to develop a new scale of Japanese version Self-Directed Learning Readiness Scale which measure the preparation of SDL.
    Researcher carried out five researches to achieve the purpose. SDLRS was used in Japanese.
    1.The influence of clinical training in nursing which effects SDLRS : Objects were forty-six nursing students in forth grade of nursing department in medical college. 85% of the student nurse raised Self-Directed Learning Readiness (SDLR) by clinical training.
    2.The study of the achievement and relevant factors of the midwifery student : Objects were sixty-six midwifery students of a vocational school. The midwifery students' achievement was related to SDLR (r=0.506, P<0.001) and self-efficacy (r=0.422, P<0.01) at the time of graduation.
    3.The study of understanding the actual situations of the self-dedication study of midwifery students in lectures and clinical training : Objects were twenty-six midwifery students of a vocational school. The SDLRS score after the lecture and the clinical training showed 8.2 points down (P<0.01).
    4.The study of understanding SDLR of midwifery students who chose obstetrics in college : Objects were nineteen students in forth grade. The students who developed SDLR after clinical training developed SDLR after practical training of obstetrics, too. The students who had not changed in SDLR, decreased SDLR after practical training of obstetrics.
    5.The study of understanding SDLR and analyzing occupational preparation action of new nurse : Objects were two hundred and thirty-tree nurses who had worked in four hospitals for one or two years. SDLR related in the new nurse's occupational preparation action (r=0.425, P<0.001).
    From the findings, researcher developed four new measures : 1)Measure of the self-evaluation sheet of midwifery students' achievement, 2)Lecture scene action observation scale, 3)Clinical training action observation scale, 4)Occupational preparation action measure of new nurse.
