    AIM: This clinical practice guideline aims to provide and recommend methods of assessing aspiration and pharyngeal residue during eating and swallowing and methods of selecting and implementing nursing care for adults to prevent the development of aspiration pneumonia through early and appropriate management of oropharyngeal dysphagia. METHODS: In April 2018, the Japan Academy of Nursing Science established the Supervisory Committee in Nursing Care Development/Standardization Committee to develop clinical practice guidelines for aspiration and pharyngeal residual assessment during eating and swallowing for nursing care. This clinical practice guideline was developed according to the Minds Manual for Guideline Development 2017, with the aim of providing a specific pathway for nurses to determine the policy for selecting management for oropharyngeal dysphagia based on research evidence and multifaceted factors including the balance of benefits and harms and patients' values. RESULTS: Based on the 10 clinical questions related to assessment by physical assessment, the Repetitive Saliva Swallowing Test, Modified Water Swallowing Test, Food Test, cervical auscultation, observation using an ultrasound diagnostic device, and an endoscope, 10 recommendations have been developed. Eight recommendations have been evaluated as the Grading of Recommendations Assessment, Development and Evaluation (GRADE) 2C, and the other two have been evaluated as no GRADE. CONCLUSION: The first reliable clinical practice guideline has been produced from an academic nursing organization that focuses on assessment for nursing care and incorporates the latest findings.

    Purpose: The aim of this study was to develop an algorithm to assess the risk of developing aspiration pneumonia.

    Subjects: Among those who were urgently admitted to the study hospital between April and August 2013 within 5 days after the onset of cerebral infarction or cerebral hemorrhage and who received conservative treatment without endotracheal intubation, we obtained consent for 160 patients to enter the study either directly from them or their families.

    Methods: We performed this study with the approval of the study ethical review board of Aichi Prefectural University and the study hospital. We collected basic data on subjects as well as relevant data regarding aspiration pneumonia from the medical records. Physical examination, intraoral observation, bacterial count, and tongue fluid volume measurements were carried out every other day from the 2nd through 14th hospital day. All subjects were grouped into either a pneumonia group or a non-pneumonia group based on definite. Subsequently, based on the algorithm, we determined the aspiration pneumonia risk and calculated the sensitivity and specificity, defining the pneumonia group as the gold standard.

    Results: One hundred and sixty subjects were classified into the pneumonia group (23 subjects; 16 developed aspiration pneumonia and 7 developed suspected aspiration pneumonia) and the non-pneumonia group (137 subjects). Fourteen subjects (87.5%) developed aspiration pneumonia by the 5th hospital day, for which we took particular note of the 2nd and 4th hospital days. Those determined as aspiration pneumonia-risk individuals by physical examination of the algorithm were 57 subjects on the 2nd hospital day and 60 subjects on the 4th hospital day. While the sensitivity and the specificity were 0.86 and 0.71 on the 2nd hospital day, they were 0.75 and 0.67 on the 4th hospital day, respectively. When there was tongue coating and secretion in the oral cavity on the 2nd hospital day, both the sensitivity and the specificity were 0.75. The inter-rater reliability on assessing each physical examination data was 82.0 to 95.3%, thus verifying reliability.

    Conclusion: We developed an algorithm for assessing the risk of aspiration pneumonia, and showed that the algorithm appears to be valid. When subjects with aspiration risk were screened using the algorithm on the 2nd hospital day, sensitivity was 0.86 and specificity was 0.71. Furthermore, when tongue coating and secretion in the oral cavity were observed, both the sensitivity and specificity were 0.75.

    超高齢社会となり、各地域で取り組まれている地域包括ケアの中でも食支援は重要である。病院に入院する場合は、摂食嚥下機能の評価や安全に摂取する方法を見出すきっかけとなる一方で、退院後の機能の変化に合わせた評価やアプローチも必要である。摂食嚥下障害者への関わりは、各病期１施設のみでは完結せず、その後も改善、維持、悪化の経過において地域での関わりが必要とされる。地域で必要な連携と教育について実践し評価を行い分析した内容を講演した。

    在宅療養者の最期の身体状況の変化における訪問看護師が行う家族への指導の状況を調査した。5名の訪問看護師を対象にインタビューを行い、その内容を7項目に分類し、カテゴリーを生成した。その結果、在宅看取り時期の救急搬送の事例は149コード、47サブカテゴリー、14カテゴリーが抽出された。これらから、訪問看護師によって最期の身体状況の変化に対して、疼痛コントロールなど在宅での対応が行えることなどを説明し、いつでも連絡できる安心への配慮をしつつ利用者・家族の受容段階に沿った説明や対応が行われていることが明らかになった。また、利用者・家族によって異なる支援状況の差や家族間の意向の違いに対処する必要性があること、看取りに関する説明の時期の予測が難しいことが課題として挙げられた。

    The purpose of this study was to clarify the cause of aspiration pneumonia in elderly people requiring long-term care living at home. The subjects of the study were 103 elderly people living in the community who needed long-term care, and data such as attributes, swallowing function, oral function, nutritional status, physical movement, and muscle strength were collected. A comparison was made between the two groups, 12 in the aspiration pneumonia onset group and 91 in the non-aspiration pneumonia onset group. The results were male, at risk of aspiration, low FOIS feeding level, low masticatory ability level, poor occlusion with both molars, inability to rattle, and significant difference in tongue pressure <20 (p < 0.05) was observed, which was more common in the aspiration pneumonia group.
    These items may cause aspiration pneumonia in the elderly living in the community, and we will continue to verify the prevention program.

    We investigated whether one's own swallowing may be aided by seeing or hearing other people swallow. Humans have a tendency to unconsciously imitate the actions of other individuals, which is known as automatic imitation. If automatic imitation of swallowing can be achieved, it would lead to the development of a simple and intuitive rehabilitation method for patients with dysphagia. We performed three experiments. Two behavioral experiments indicated that automatic imitation of swallowing was elicited when the swallowing sound was heard, rather than when a swallowing motion was seen. A brain imaging study suggested the left inferior frontal gyrus and the right superior temporal gyrus were involved in automatic imitation elicited by the sound of swallowing.
