    In order for people with dementia to use their habits and techniques abundantly in their daily lives, it is necessary for the people around them to communicate with each other to elicit them. In this study, we created a "communication education program that promotes the daily life of people with dementia" and interviewed some local residents.
    The results are as follows :”The explanation of the illustration is easy to understand”” I understand that by saying words to invite orsuggest to that person with dementia , I can elicit the lifestyle and habits that people with dementia have. I don't know if I can, but I'll try”. These resalts indicate that the purpose of this educational program would be transmitted to local residents.

    The first study revealed that the observations advanced practitioners of nursing obtain from elderly patients range from physical aspects, including neurological symptoms, to life history. The second study revealed that nursing practitioners perceived "short-term memory," "degree of dementia," "attention," "delirium scale," "checking disorientation," and "history of stroke: delirium during hospitalization" as particularly important. We followed the factors affecting the state of delirium in elderly stroke patients shown in previous studies to come up with a draft. The risk assessment tool was considered to be practical in two phases, one on the first day of illness and the other after the second day of illness, consisting of observations for risk assessment of the onset of delirium and for the creation of a care plan to prevent delirium/relieve symptoms.

    The aim of this study was to develop an educational program of communicational skills to elicit daily life behaviors among elderly people with dementia. A characteristic of this educational program is to use the specific action and this evidence to elicit daily life behaviors among elderly people with dementia we found by ethnomethodological method. Another characteristic of this educational program includes to discuss cases by participants of this program.
    We provided this educational program to nurses and care workers. And they gave a good evaluation to the program.

    The purpose of this study is to implement a cognitive function-preserving care protocol devised for the prevention of delirium in elderly patients undergoing acute treatment and to clarify changes in delirium status among elderly patients. The study was carried out at a hospital that treats elderly patients. Before implementation, a study sessions focused on delirium knowledge, this care protocol, and other matters was held for the nurses cooperating with the study.
    The protocol was centered on simple checks of cognitive and auditory-visual functions together with intervention based on Reality Orientation (RO). Going forward, there is a need to further refine the contents of the intervention as well as the conditions for application of this care protocol among elderly patients.

    In the present study, an educational program for promoting multidisciplinary care and improving the care skills of nurses in delirium risk management at acute hospitals was developed. First, the activities of pioneering delirium management teams in Japan were investigated, and the process of establishment of multidisciplinary teams was elucidated. Then, a DVD teaching material for nurses aimed at facilitating the introduction of delirium assessment tools in clinical settings was created. Finally, a delirium risk management program comprising the three pillars of establishment of organizational structures for promoting delirium management, establishment of systems for delirium management, and staff education regarding delirium care was created. It is necessary in the future to apply the present program to clinical settings, and to assess its usefulness and effectiveness.

    The purpose of this study was to develop delirium care management for elderly patients in acute hospital settings. The research program was included to develop and administrate an educational program for nurses, and to assist delirium care leaders in the efforts to improve delirium care at participating wards. One year after the start of the program, the development process and outcomes of the program were evaluated by quantitative and qualitative analyses. The findings showed that nurses were improved awareness of the needs of delirium sufferers, resulting in changes in care quality. To improve the delirium care quality provided at general hospitals, it is necessary to organize the tasks involved in the wards, improve instructions to nurses in how to use the evaluation tools, and simplify recoding methods to allow nurses to concentrate on the delirium care, as well as to support and encourage the efforts of the nurses.

    The purposes of this study is to examine a method that, from reactions displayed by the elderly to the environmental factors, predicts the risk of onset, and reduces the risk of onset due to the environment.
    In the first stage, the hospitalized elderly patients' level of recognition of environmental factors involved in the onset of delirium was investigated. In the second stage, the target group was divided into those with onset of delirium and those without onset of delirium and a comparative verification process was conducted.
    The results of this study showed that differences were observed(<. 05) between the two participant groups regarding the physical and human environment in the following categories :
    ●The location of the room
    ●The frequency of visits of the nurses
    ●The presence or absence of medical examinations that exacerbate stress
    ●Interaction with other patients
    ●The presence of elements that heighten anxiety
    These differences are thought to be connected to the onset of delirium.

    The purpose of the present study is to clarify the realities of the factor that relates to the delirium onset and the delirium onset that centers on sleep. The object person is an aged cerebral infarction patient who receives the acute treatment. Characteristics, sleep state and the delirium onset related factors were collected during 1to7 hospitalization days. The NEECHAM confusion scale Japanese version was used. As for the significant difference, the delirium onset person was not thought that the average sleeping time the day before was short compared with the person who did not have onset though tended.CRP,pain,orientation level,restrain were significant. These factors were different depending on time.

    研究目的:
    本研究の目的は,脳卒中急性期にあり,かつ内科的治療を受ける高齢患者のせん妄発症に対する看護師の臨床予測(Clinical Forethought)の構造を明らかにすることである.本研究では,「臨床予測」の操作的定義を「ある臨床診断,あるいは状態増の異変や転帰に対する予見の総称」とした.
    18年度の進捗状況:S市内の脳神経外科専門病院に勤務する看護師5名に研究参加同意を得て,インタビュを行った.また,遂語録を作成し,分析を進めている.
    結果と考察:
    <対象者の属性>S市内の脳神経外科専門病院または脳神経外科病棟を有する施設,計3施設に勤務する20名の看護師にインタビューを行い,准看護師1名を除く19名を分析対象者とした.19名(女性17名/89.5%,男性2名/10.5%)の平均年齢は29.0±7.34歳であった.看護師の経験年数は平均6.94±6.64年で,そのうち脳神経外科での看護経験年数は,平均5.11±4.12年であった.看護師の経験年数3年未満は7名(36.8%),3年以上5年未満が5名(26.3%),5年以上7年未満が3名(15.8%),7年以上が4名(21.1%)であった.看護の最終学歴は,12名(63.2%)が専門学校,3名(15.8%)が4年制大学,そして,短期大学,進学コースの卒業がそれぞれ2名(10.5%)であった<看護師経験年数7年以上の看護師のせん妄発症に対する臨床予測>看護師経験年数7年4名(全員女性,平均年齢40.8歳,脳神経外科看護の平均経験年数9.3年)の臨床予測に基づく観察項目は,「脳梗塞の悪化」,「脳梗塞による障害」,「安全・安楽な入院生活」に関連する事柄に大別された.また,観察項目の中で最も注目しているものは「意識の低下」などの脳梗塞の主症状であった.「環境今の不適応」「精神的動揺」などのせん妄発症への注目度は高くなかった.せん妄発症を察知知るために着目している事柄は,患者特性「高齢」,「認知症がある」などで,発症を示す患者の変化「目つき」,「仕草」,「ADLの手順」などであった.これらの観察の体得に影響したものに「全員が「自己の経験」を挙げ,他は「文献」,「他の看護師の経験」を加えた.以上より,急性期の臨床予測は脳梗塞の症状が中心であった.また,せん妄発症を察知するために様々な着目点を経験的に体得しているが,せん妄の知識体系においての不足が示唆された.
    今後はさらに分析を進め,経験年数による臨床予測の差異を検討する.
