    OBJECTIVES: Subglottic cancer is a rare malignancy for which limited data on its epidemiology, prognosis, recurrence, and laryngeal preservation are available. This multicenter, retrospective study aimed to elucidate the treatment outcomes of subglottic cancer compared with glottic and supraglottic cancers. MATERIAL AND METHODS: Sixty-six patients with subglottic cancer were identified among 1763 patients with laryngeal cancer treated with curative-intent surgery or (chemo)radiotherapy at 13 hospitals in northern Japan. Patients' characteristics, treatment modalities, recurrence patterns, and prognosis, including disease-free survival (DFS), overall survival (OS), disease-specific survival (DSS), and laryngeal-preservation survival (LPS), were compared across the three primary tumor subsites. Outcomes of subglottic cancer were further analyzed according to initial treatment. RESULTS: Compared with glottic and supraglottic cancers, subglottic cancer presented with more advanced T status, higher rates of total laryngectomy and recurrence, and poorer DFS, OS, DSS, and LPS. Analysis by stage indicated that subglottic cancer tended to show poorer DFS in stage III-IV cases. In subglottic cancer, (chemo)radiotherapy was effective for T1-2 disease, whereas T3-4 cases had a high local recurrence rate. Total laryngectomy provided good local control in T3-4 cases, but it was frequently followed by nodal recurrence and distant metastasis. CONCLUSION: Subglottic cancer demonstrated a more locally advanced presentation and poorer prognosis than other laryngeal subsites, a trend also observed in stage III-IV disease. Though initial surgery provided good local control in advanced subglottic cancer, it was associated with high rates of nodal recurrence and distant metastasis.

    OBJECTIVES: The predictive value of programmed death-ligand 1 (PD-L1) expression for nivolumab efficacy in head and neck squamous cell carcinoma (HNSCC) remains controversial, partly due to variability among antibody clones. This study compared the SP142 and 28-8 PD-L1 antibody clones using multiple scoring systems to identify the most informative parameters for predicting nivolumab efficacy. METHODS: We retrospectively analyzed 26 patients with recurrent or metastatic HNSCC treated with nivolumab. PD-L1 expression was evaluated using SP142 and 28-8 antibody clones and quantified by four scoring systems: tumor proportion score (TPS), combined positive score (CPS), tumor cell (TC) score, and immune cell (IC) score. Associations between PD-L1 indices and clinical outcomes, including progression-free survival (PFS) and overall survival (OS), were assessed. RESULTS: The SP142 clone yielded lower tumor-associated PD-L1 scores (TPS and TC score) and higher immune cell-associated scores (IC score) compared with the 28-8 clone. None of the PD-L1 parameters assessed using SP142 were significantly associated with PFS or OS. In contrast, the 28-8 clone demonstrated clear predictive value: CPS assessed with 28-8 was significantly associated with both improved PFS and OS, while TPS and TC score assessed with 28-8 predicted PFS but not OS. These findings indicate that SP142 may underestimate tumor cell PD-L1 expression, whereas 28-8 provides a more balanced assessment of PD-L1 distribution across tumor and immune compartments. CONCLUSION: Clone selection is critical in PD-L1 immunohistochemistry, and 28-8-based CPS appears to be a practical biomarker for guiding nivolumab treatment in this setting. EVIDENCE LEVEL: Level 5.

    OBJECTIVE: Otitis media with effusion (OME) is common in nasopharyngeal carcinoma (NPC), but its pre- and post-treatment forms may represent distinct clinical entities. This study aimed to compare tumor- and host-related risk factors between pre- and post-treatment OME in NPC. METHODS: This retrospective cohort study included 31 patients with newly diagnosed NPC treated at a tertiary referral center. Clinical, radiologic, and otologic data were reviewed. Associations between clinical parameters and the presence of pre- and post-treatment OME were evaluated. Tumor-related factors, including T classification and magnetic resonance imaging-defined involvement of the tensor veli palatini muscle (TVPM), as well as host-related factors such as mastoid pneumatization, were analyzed. RESULTS: Pre-treatment OME was observed in 19 of 29 patients (66%) and was significantly associated with advanced primary tumor classification (T2-4 vs T1; p = 0.03) and involvement of the TVPM (p = 0.01). In contrast, post-treatment OME occurred in 14 of 31 patients (45%) and showed no significant association with tumor extent, nodal status, or TVPM involvement. Instead, poor mastoid pneumatization was significantly associated with post-treatment OME (p = 0.05). Persistent OME before and after treatment was observed in 10 of 29 patients (35%), indicating that tumor control does not necessarily result in resolution of middle ear effusion. CONCLUSION: OME in NPC comprises biologically distinct entities before and after treatment. Pre-treatment OME is primarily driven by tumor-related anatomical factors, whereas post-treatment OME is more strongly associated with host-related factors including mastoid pneumatization.

    BACKGROUND: Regarding concurrent chemoradiotherapy (CCRT), the combination of docetaxel and cisplatin (DP regimen) is a promising option for treating head and neck squamous cell carcinoma (HNSCC) with a relatively low dose of cisplatin; however, its non-inferiority to other chemotherapies in efficacy and tolerability remains unclear. METHODS: In this retrospective multi-institutional study, the efficacy and safety of the DP regimen were compared with those of other chemotherapy regimens in patients who underwent CCRT for HNSCC. Overall survival, progression-free survival, and adverse effects-including estimated glomerular filtration rate (eGFR)-were evaluated as outcome measures. RESULTS: Study results showed that a total of 211 patients were included. The prevalence of oropharyngeal, hypopharyngeal, and laryngeal cancer was 44%, 33%, and 17%, respectively. Overall response and recurrence rates were comparable between the DP regimen and high-dose CDDP alone. Although overall and progression-free survival tended to be longer with the DP regimen than with high-dose CDDP, the differences were not statistically significant. Neutropenia was more frequently observed with the DP regimen, but the chemotherapy completion rate was comparable to that of high-dose CDDP alone. Regarding renal function, eGFR significantly decreased with high-dose CDDP but not with the DP regimen. CONCLUSIONS: A combination of docetaxel and cisplatin in concurrent chemoradiotherapy was a favorable option for treating HNSCC with acceptable efficacy and manageable toxicity.

    INTRODUCTION: Netherton syndrome is a rare autosomal recessive disorder characterized by congenital ichthyosis, hair shaft abnormalities, atopic diathesis, and a broad spectrum of immunologic dysfunction. Although cutaneous malignancies are uncommon, affected individuals may develop early-onset tumors that can pose diagnostic and therapeutic challenges. CASE PRESENTATION: We report the case of a 33-year-old woman with Netherton syndrome who presented with persistent left otorrhea and a granulomatous mass in the external auditory canal. Imaging demonstrated a soft-tissue lesion with focal bony erosion and suspected dural involvement, accompanied by fluorodeoxyglucose uptake in a cervical lymph node. Biopsy confirmed squamous cell carcinoma (SCC), staged as T4N1M0. The patient underwent superselective intra-arterial cisplatin infusion combined with radiotherapy, resulting in complete remission of both the primary lesion and nodal metastasis, with no evidence of recurrence over 4 years. Including this case, 9 patients with Netherton syndrome and cutaneous malignancies have been reported, with tumors frequently arising at a young age and showing variable HPV status. Interpretation of SCC antigen may be complicated by elevations associated with atopic dermatitis. CONCLUSION: This case highlights the elevated risk of cutaneous malignancies in Netherton syndrome and underscores the need for long-term surveillance and multidisciplinary care.

    Superinfections with various bacteria exacerbate the symptoms of coronavirus disease 2019 (COVID-19). Bilateral peritonsillar abscesses are rare and could be fatal, and their occurrence after COVID-19 has not been reported. A 20-year-old woman visited our hospital with a sore throat. The patient was diagnosed with COVID-19 infection, with no tonsillitis or peritonsillar abscess. Five days later, she returned to our department because of the deterioration of a sore throat. The patient was diagnosed with bilateral peritonsillar abscesses and admitted to the hospital for abscess drainage and antibiotics, which relieved her symptoms in seven days. Although a sore throat is a common symptom of COVID-19, clinicians should be aware of the possibility of other upper airway diseases, including bilateral peritonsillar abscess, during COVID-19 treatment.

    BACKGROUND Malignant lymphomas can occur at various sites. Hypopharyngeal tumors are at risk for airway obstruction and require rapid diagnosis and treatment. Most hypopharyngeal malignancies are squamous cell carcinomas; other tumors are rare. To date, only a few cases of malignant hypopharyngeal lymphoma have been reported, and its specific characteristics are unknown. Herein, we report a case of right hypopharyngeal diffuse large B-cell lymphoma (DLBCL) in a 74-year-old man with dysphagia. CASE REPORT A 74-year-old man presented to our hospital with dysphagia. He had no relevant medical history. Endoscopic examination revealed a right hypopharyngeal tumor. The surface of the tumor was smooth, with no evidence of hemorrhage. Computed tomography revealed a 40-mm mass located in the hypopharynx. We performed a tracheotomy and biopsy of the tumor. Histopathological examination revealed a diffuse proliferation of large atypical B cells with negative staining for Epstein-Barr virus by in situ hybridization. Immunohistochemical staining was positive for CD20 but negative for CD3 and CD10. The patient was administered chemotherapy. The tumor reduced in size, and the patient recovered completely. During the two-year follow up, no recurrence of cancer was observed. CONCLUSIONS Although most hypopharyngeal tumors are squamous cell carcinomas (SCCs), the possibility of other types of tumors should also be considered. Malignant lymphoma of the hypopharynx is rare, and more cases need to be studied and reported in the future.

    A coin cell lithium battery is a common foreign body that can become lodged in the pediatric pharyngoesophageal junction. Because the voltage of such batteries is relatively high, their rapid removal is necessary to avoid mucosal necrosis. Despite being the initial choice for removal, flexible endoscopy cannot remove such foreign bodies from the esophagus. Various removal methods, including rigid esophagoscopy, should be considered for removing lithium coin cell batteries. The transcervical approach is feasible for removing esophageal foreign bodies, but it carries the risk of complications such as esophageal stenosis. Here we report a case of lithium coin battery ingestion that was successfully removed using a rigid esophagoscope. A 2-year-old girl was referred to a local doctor with cough and general fatigue. Chest X-ray and flexible endoscopy revealed a coin cell lithium battery stuck in the pharyngoesophageal junction, but it could not be removed. The foreign body was removed using Nishihata forceps through a rigid esophagoscope under general anesthesia.
